How to Request Records

You have the legal right to request access to your health care records maintained by this practice.

To make a records request:

• Submit a written request including:

  • Your full name

  • Date of birth

  • The specific records you are requesting

  • Dates of service (if known)

We will process your request in accordance with applicable Texas law and federal privacy regulations (HIPAA). Please note, if you request a paper copy of your records, a fee will be charged.
